Admission - discharge conditions

INTERNMENT

Access to the clinic and admissions are made only through appointments made in advance.

What will you be able to have with you at the time of your hospitalization?

The clinic provides all the items you need   during the hospitalization, so you will bring with you only the documents (act of identity   you, analysis), a personal hygiene kit(toothbrush and toothpaste, deodorant, clipper, razor) and personal electronics (tablet, phone, laptop).

Documents required for the admission of private patients: identity card, passport, birth certificate, if it is about minors are 14 years old.

As for the medical assistance in the clinic, it will be done in day hospitalization or continuous hospitalization .

Before admission, the patient must fill in these documents*:

  • Patient record (can be downloaded from the site at any time, it is preferable to be completed before arriving in the clinic)
  • General consent form
  • Specific consent form by type of procedure
  • Consent form specific to the protection of personal data

*Patients cannot be hospitalized if they do not fill in and sign the mentioned documents.

 DISCHARGE

Discharge is made according to the recommendations of the doctor, depending on the procedure and the progress of the healing process . At the same   time,it is done through the office of admission/discharge daily or at the request of patients or legal relatives, by filling in the discharge form on request on their own responsibility.

At the time of  discharge, patients will have their health card with them and will receive these documents:

  • The medical prescription – if the doctor prescribes a drug treatment
  • Exit ticket/ medical letter (if necessary)
  • Leave certificate (if necessary)
  • Expense statement
